To join, fill out this application in pdf format. You will need Adobe's Acrobat Reader installed on your PC to open and print the application. Download the latest version of Acrobat Reader at Adobe's website here. If you prefer using the application in MS Word format, download it here. To open the Word document and print it out, you must have MS Word installed on your PC or the MS Word viewer which can be download here.
Print your information clearly on the application form, sign the application, and submit it via US Postal mail with your payment (no cash please) to the stated address in the application. You may also electronically complete the information on the Word formatted application (You must have MS Word installed on your PC in order to fill out the MS Word-formatted form electronically). The application has 4 pages so please be sure to complete each page. Note that the annual dues are $30.00 per familiy, with a $5.00 early bird discount if the application is postmarked by June 1 of the upcoming fiscal year. A $5.00 addtional fee is added for those requesting their newsletter be sent via US Postal mail. A LATE penalty fee for applications submitted AFTER Sept 15 of the fiscal year is $20.00.
